Waiver and Release Agreement - deemed as agreed upon Booking Confirmation: "I, hereby declare that I am the legal owner of the dog(s) named in the booking confirmation and affirm the following: 1. My dog has not been exposed to rabies, distemper, parvovirus, parainfluenza, or any other contagious disease within the past thirty (30) days. I further confirm that my dog's vacinations are up-to-date. Proof of current vaccinations is uploaded for records. 2. Acknowledgement of Risks: I understand that boarding involves inherent risks, and despite best efforts, it is a home care practice that does not necessary included 24-hour human supervision. I authorize Kelly (and/or her helpers) to take any necessary actions for the safety, health, and well-being of my dog, including seeking professional treatment. 3. Medical Attention: Every effort will be made to provide a safe environment. In the event of a medical issue, I will be contacted immediately for guidance on where to take my dog. If unreachable, Kelly (and/or her helpers) is authorized to transport my dog to the Emergency Animal Clinic. 4. Responsibility for Damages and Injuries: I accept sole responsibility for any damage or injury caused by my dog while boarded. This includes costs related to injuries to caregivers, other clients, animals, or damage to facilities. I hereby waive any claims, actions, or demands of any nature, foreseen or unforeseen, against Kelly's (and/or her helpers) Dog Services arising during pick-up, transportation, drop-off, and stay at the facilities. 5. Discretionary Acceptance: I acknowledge that my dog’s acceptance into boarding is at the discretion of Kelly's (and/or her helpers') Dig Services, with the right to refuse service at any time for any reason. If my dog exhibits aggression or biting behavior, Kelly's (and/or her helpers') Dog Services reserves the right to refuse service. 6. Release of Liability: Upon booking confirmation, I accept full responsibility for my dog's actions while boarded. I release Kelly’s (and/or her helpers') Dog Services from any and all claims of liability. 7. Assumption of Risk: I have carefully read and understood the contents of this Waiver and Release Agreement. I acknowledge that it includes an assumption of the risks associated with negligence and liability on the part of Kelly's (and/or her helpers') Dog Services. 8. Reliance on Waiver: I recognize that Kelly's (and/or her helpers') Dog Services is materially relying on this waiver. I understand that my dog(s) may engage in play with other dogs during their stay in the daycare, boarding, and at Kelly’s (and/or her helpers') Dog Services. I confirm that I have thoroughly read and agree to comply with all the terms outlined above. "
