What Lois would like to know about your dog
I would like to know of medical issues, physical condition, medication, unique behaviors, likes and dislikes, sleep preferences, motivation incentives and age. Also, he if is up to date on vaccinations such as Rabies and Bordetella. Does he/she have aggressive tendencies to dog, cats, or humans. If your dog is a large breed, i.e., German Shepherd, Mastiff or Pittbull, etc..., do they interact well with toy and miniature breeds? If your pet has fleas, will you allow treatment with CapStar and follow up with flea preventative to be reimbursed by you, the client.
