What Kimberly would like to know about your dog

I would like to know the dog's history of boarding, daycare and grooming. (Has the dog been to a boarding facility or home boarding environment before? Is the dog friendly with children or prefer adult attention? All dogs bite if put in an uncomfortable position, but the question for this is do you know of an incident when this dog has bitten someone or another animal and please describe the situation. Describe your dog's personality. Is your dog a free feeder (leave the food out and the dog will nibble at it at will or eat throughout the day) or has a regular appetite, eating 1-3 meals a day? Does the dogs have medications and what kind of medications are they? Is the dog friendly to people and other dogs? I have no breed preferences but prefer not to board giant dogs. I mostly board small- medium/large dogs.
What kind of tempermant does your dog exhibit in group play environments, such as the dog park, doggy day care, grooming, or boarding situations? How long have you had your dog? What vet do you use and please provide the vacciantion paperwork. All dogs must be current on bordetella, rabies, distemper parvo, and canine influenza through a veterinarian vaccination record. Your dog must be booked for a free play date, called a socialization before boarding, or being involved in continuous day care in order for me to assess whether the dog will fit into the pack.
