What is your dog’s meal schedule?
Is the dog used to being left alone? How long? What times?
What time does the dog usually sleep and wake up, where do they sleep? Are they crate trained?
Is your dog allowed on furniture at home?
What is the dog’s usual routine for walks, exercise and outings?
What kind of collar does the dog wear for walks? Do they have a harness or fixed length leash?
Does your dog have any favorite toys or games?
Special Needs or Concerns
Does your dog have health issues? (e.g. arthritis, allergies, etc.)
Does the dog need special additive in their food?
Does the dog take medication? If so, how is it administered?
