How much exercise does your dog require?
What activities does your dog love?
When, and how much food does your dog eat?
Does your dog have any allergies?
Does your dog like people? Other dogs? Other animals in general?
What are some idiosyncrasies about your dog? (Escape artist, digger, jumper, talker, kisser, etc.)
Is your dog crate trained/free roaming without supervision?
Does your dog react to certain commands? Does your dog need improvement on any commands?
