When I do a Meet & Greet for any dog staying at my home, I give the owner a form to fill out, which asks for all the relevant information about the dog (age, medical, etc.) but also behavioral things. It also has a section on information about the owner, which includes their address, phone numbers, email address, and emergency contact. The form is returned to me when the dog is dropped off for boarding.

No one has ever refused to provide the information.

That's an awesome idea, Karen! Would you mind sharing your form? I'd like to do something similar.

Just do some Googling for them. They're usually online for boarding facilities/kennels. I used those to create my own Word doc.

OMG, I just Googled and now find there's an entire industry of people providing forms on etsy and other places. You'll find so many with excellent ideas. I may even revise mine with what I saw.

Yes you can ask. If this is for housesitting, then you’d want to do a meet and greet in their neighborhood and then see where the pet’s stuff is located inside and any areas you might access. If the pet is coming to your home, they may not want to provide their address but might tell you major cross streets.
