I am looking for an initial Meet & Greet form for dogs that I may be pet sitting for. Seems like there was something on Rover but I can't locate this any longer.
Anyone have or remember this form?

Rover never had a form and it is really difficult finding the checklist unless you know specifically what to Google. There is no easy way to find it through Help or Sitter Resources either. Totally pathetic site administration IMO, but the list of questions is still available here:

I made up my own Word form from questions I found at boarding places all over the country. One side of the form is for the basic info on the dog and owner (medical and emergency stuff) and the other is for behavioral things, plus space for notes.
The Rover checklist should be a decent starting point if you want to customize.
Oh yes, some people create online forms (Google docs), but I like to keep it on paper in a binder for all my guest dogs, so I can make notes and they're easy to reference.
