Hi, is there a questionnaire on this site somewhere for sitters to have owners fill out about dog routine? what time they eat, are there meds, are there any situations that spook the dog, how to you calm them or comfort them. Favorite toys... etc? I can't seem to find one

I have put together a form with basic and necessary info (including medical, emergency contacts and behavioral things) that owners fill in and bring when they drop off their dog for the first time. I give them the blank form at the M&G. I prefer having it on paper so I can make notes to reference. However, there are some sitters who might use an online form. Whatever works best for you.
River does have a m&g form not just for sitters but for owners as well. But like the sitter below, I crafted my own version based on my experiences and feedback. Feel free to contact me for access to any of the above mentioned material.
