I can't find the pet sitter questionnaire for meet and greet where pet sitter has a list of questions to ask pet owner about their dog, schedule, habits, etc.?

Question: Is this questionnaire something I should send potential clients?

I have my own Dog Info Sheet which I give to clients at the M&G and ask them to fill out and bring back when they drop off their dog. I keep this record for all dogs and can make notes on the record during the stay.

I also have my own form for the pet parent to fill out and make notes during the stay. I also give the pet parent a detail list of information of my process and procedures in case something was forgotten during the meet & greet.
