Additional charge for senior/special needs dog?

I have a sitting coming up for a dog that requires it's back legs to be held and carried for him to walk anywhere, and when he goes outside to the bathroom. Even with doing this it sounds like you may need to clean him up after every potty time. I am completely fine with caring for him and one other dog they have, but I am starting to think this will be quite a bit of extra work (we will have them for 6 days). Is it appropriate to have an additional cost for care of this kind? If so, what do you all charge?

It would be fair to charge $30 more per day for a small dog, $50 for a large dog. If they are long hair like a pom or German shepherd, it takes a lot of time and care

We have run into similar issues and decided that if an adult dog takes extra care we charge them the puppy rate, as there is a similar level of care. I have seen other sitters who charge a few dollars extra if the dog needs to have regular pills, that could cover any medication-related stress as well.
