Has anyone developed a questionnaire for dog boarding?

We are looking for a through questionnaire to have owners fill out for dog boarding. Does anyone have one to share?

I developed mine from reviewing questionnaires used by boarding facilities that I found online. I took bits and pieces from a variety of places and typed it up as Word doc, which I print up and give to clients at the M&G and they return to me filled out when they drop off their dog.

I have found a very effective way to handle this same issue. After speaking with a client during the meet and greet part, I asked several questions concerning them being boarded at my house. Just so I can get a feel for how the dog is. Then if they do accept and want to book me, that’s when I send a couple questions in the message center to them. Question such as does the dog have any quirks? Does a dog have any food or bone or toy aggression? Any allergies? Stuff along those lines. Then a day or two before the dog comes to my house, I messaged them telling them I’m excited to have their dog over in the next couple days and if they would be willing to write me out a little note about stuff we’ve talked about. Anything that I would need to know to make their dog more comfortable at my house. I’ve even had typed out full page letters. I was very impressed and I even remarked to the owner about how thorough he was and how grateful I am for that. Last client I had had a three ring notebook full of everything from microchip info to vet records etc. I hang onto these letters for future rebooking. 😉

Absolutely! I require meet-and-greets for all of my new clients, regardless of the service they need, so we can chat about specific questions I keep printed up on a clipboard for each client. Then I keep my files in a Rover folder for future bookings, which is incredibly helpful. I start by confirming dates and times, write the pet's basic info like weight and age (usually in the pet's Rover profile), and ask for the pet's backstory. We also discuss how the pet does with cats, dogs, people, and children; where the pet is used to sleeping at night (crate trained? in bed with you?); medical conditions/allergies; triggers/behavioral issues/places to avoid touching; how to access the home (for my drop-ins); updates I'll send; whether they are new to Rover (you can give a $20 discount code to new folks that won't impact your own earnings); vet and emergency contact info; and then the largest section is for the basic care instructions, like food, toys, and walks (for boarding I would ask what schedule the dog is used to and will expect). My questions differ slightly depending on the pet and service. When I offered boarding, the most helpful questions for me were the dog's normal schedule and where he/she was used to sleeping. I also asked how long the dog was used to being alone/could be left (and whether it would need to be crated). Don't be afraid to say no if you find during the meet-and-greet that the dog is probably not going to be a great fit--you, the owner, and the dog will all benefit from ensuring they find the right match.

I have a questionnair I complete during the required meet and greet for new customers. It covers the below: Customer name, contact info, and emergency contacts Basic pet info (name, breed, age, etc.) Care info (house trained? crate refined? sleep arrangements? Walks? Allowed on furniture?) Feeding schedule Food allergies Vet info Medications Medical conditions and allergies Any behavioral.issues Any fears the pet has Has pet bitten a human or another animal?

I like the Rover emails, ie: a Meet & Greet mailer that covers every aspect of what the sitter needs from the pet parent. Very helpful! I make notes on the meet & greet. And prior to the M&G, I encourage the pet owner to continue to fill out the Rover app on their pet and include a picture.
