Veterinary authorization?

Does anyone have a recommendation for wording? I'd like to make up a form for owners to sign that shows I have permission to take their pet to the vet in case of emergency.

Here's mine: "In the event that my pet(s) need medical attention during their stay, I authorize Leighann H. to make such decisions in my absence. I understand that every effort will be made to notify me prior to taking this step. I also understand that I will remain financially responsible and liable for any and all of my pets(s) medical expenses."

I wrote my own, since I also included a line to allow me to check vaccination status, and another bit allowing the vet to release to someone else if the dog is unable to go home with me.

You can also have the client call their vet ahead of time to let them know they'll be out of town and their dog will be with a sitter.

Mine is very similar to Leighanns. I also include permission to check vaccines and have a "per Rovers policy" line, as I always give owners a copy of Rovers Insurance policy at the meet and greet. A Veterinary release is a great thing, I'm not sure about other states but in Florida a Vet will not treat or release ANY medical or vaccine imformation without the owners consent.

Here is the wording on mine - I paraphrased one I found on another pet site. I include the Pet Owner's identification at the top and my identification info at the bottom. I also get their vet contact information.

T O WHOM IT MAY CON CER N I hereby authorize the attending veterinarian to treat any of my pets as listed on the Pet Information sheet and I accept full responsibility for all fees and charges (limited to $____) incurred in the treatment of any of my pets.

The Pet Sitter is authorized to transport my pet(s) to and from the veterinary clinic for treatment or to request "on-site" treatment if deemed necessary. If I cannot be reached in case of an emergency, the Sitter shall act on my behalf to authorize any treatment excluding euthanasia.

I googled Vet consent form and picked the one that I liked. There are a ton of them on the internet. I have had to use the form at the vet and it really helps. Happy sitting, Amber G
