What to put on job applications?

Hi all! :) I've been fortunate enough to be exclusively sitting for Rover since graduating from college, but I'd like to apply for a temporary position at my local animal shelter and I'm finding it a little awkward as to how to put down my information from Rover. I'm guessing for the Job title I would say I'm a pet sitter/independent contractor? There are also spaces for an address, telephone, and supervisor. What should I put in those?

Not sure what exactly this specific application looks like, but since you are not supervised, I think it's completely fine to put yourself down or write 'self-employed'. Lots of places will probably understand what you mean, and they should allow some blank or 'n/a' answers.

If there's space for text, I'd recommend you point out that you are 5-star rated by your clients! You could maybe even link your profile for them.

We are considered independent contractors for Rover, which means we are self-employed. If you operate using a business name, I would use that (like I use City Dogs Dog Sitting), and you can list your home address/phone, though I would be sure to list that you are affiliated with Rover, since that's verifiable.
