There can be many medical reasons, including insect bites, and allergies. Your dog needs to be looked at by his regular veterinarian.

My dog began scratching her face, especially around her eyes, when she was 11 months old. She would scratch her face with her paws to the point where she began losing hair around her eyes and looked like a raccoon. We took her to our vet who told us that dogs usually begin developing allergies around a year old. So we tried an allergy injection shot that didn't work and then advanced to oral medication (Apoquel). It worked within a few days and she stopped scratching her eyes and regrew the hair around that area. Unfortunately, she does have to take it every day because we noticed she'll start scratching if she doesn't get it even for one day.
