Is there a checklist or form to capture info about the pet and the home?

I'm looking for a standard form to use that captures all pertinent information about the pet: (where leashes, bags are kept, fill water bowl to the top, where treats are, unique things about the pet, ie, don't touch their feet, favorite toy), etc. Also a form to capture info about the owners: cell phone #, alarm code, water plants, etc.

I'm getting ready to meet and greet my first client and want to be prepared if they hire me :)

Kudos to you for being proactive! But I'm not aware of any particular forms ready-made. I made my own. You could do an online search of petsitting businesses to see if any of them have downloadable informational forms meant for clients. I wouldn't use them verbatim, but you could use them as a jumping off point for designing your own forms.

I don't use a form, but always take a notebook with me to the M&G to record that information. It really helps to have notes in my own phrasing, since many clients become repeats. Then I don't have to ask them the same stuff they covered at first. After the stay, I put the most important reminders in the dog notes section in Rover. I only do drop-ins and dog walking, so am always in my clients' homes. The most important info to me, besides the pertinents about the pet, is lock and key info. Nothing worse than having a lock out or a weird lock you can't open! Practice opening the lock in the owner's presence at the M&G! More often than not, you'll hear something like "oh yeah, you have to jiggle it a little..." Or whatever!
