The dog still smells and we had no idea about it in advance of their grandmother dropping him off. What should I do now ?

Tell them what you noticed and the details from conversation with grandmother if she shared any. Ask them what they’d like you to do. Some options you may discuss include: having their vet or groomer handle it, or (if you’re up for it) washing yourself with homemade or commercially bought solution.
Basic De-Skunk shampoo for dogs=
1 quart of 3% hydrogen peroxide solution
1/4 cup of baking soda
1 teaspoon of liquid dishwashing soap
To be followed with Dog shampoo
Oh and anything dog has rubbed against in your home likely needs to be laundered with Baking Soda & detergent.
