Sitter is asking me to fill out the extended profile for my dog but I can't find it?

I am trying to book day care with a sitter, but she requires all dogs need to have their extended profiles filled out for her information. I can't find where to do this, does anyone know where she means?

Actually, there are 2 sections (why two, I don't know): Profile and Extended Profile. Unfortunately, I cannot tell you where they appear, since I never see the dog owner's side of things.
