How do I get the extra buttons/ribbons on my profile?

Take and pass the Rover 101 quiz. Get first aid and CPR trained and add the badge, those are the only ones currently available.
In addition to what Walt said, if you donate a portion of your Rover earnings to a charity you get a badge for that.
Edit: Another way to get a badge is to join one of the pet sitting organizations recognized by Rover.
