I agree that water intake needs to be monitored and your dog may need to be drinking more. Dogs should definitely be going more than once a day. I feel it continues happening once you monitor intake, it may be time for a vet trip. It could be an indicator of a serious issue, like kidney trouble.
depends how old you pet is and where it came from. It may just have been accidentally trained that way if your not home a lot or it's previous owners were that way. It honestly depends on so many factors. If you are really concerned about it take them to the vet.

Your dog must be very well house trained however it sounds like they are not drinking enough water and could be dehydrated.
