Puppy does fine in the crate but pees in the house?

I have a 13 week old puppy. He is doing great with crate training and does not have accidents unless I give him too much room in the crate. He can also sleep through the night without needing to go out. When I am home, we go out very frequently (sometimes every hour or sooner) and I give him lavish praise and treats for going outside. However, he still pees in the house. Sometimes these accidents happen even within 10 minutes of coming in from a successful potty break. He just went to the vet and has a clean bill of health. He is obviously capable of holding it longer, but is choosing not to. Is this an emotional or behavioral issue? There does not seem to be a pattern of where he is going, so I don't believe the issue is that he smells the pee from the last place he marked. Any advice on how to manage this would be appreciated!

🐶ANY SUGGESTIONS ARE WELCOME PLESSE HELP US!!🐶 My 1yr old shitzu dog that’s doing the same exact thing as yours EXACTLY but mine is a spayed female 1yr old & we’ve been potty training for 1 solid yr solid!! I was using AllStar Brand PeeWee Pads... She definitely does not like the smell of the attractant on that brand so I switch pads two Harts brand purple pads smell sweet but she’s attempted a couple times but still the battle goes on!! She holds her you urine all night long so I know she can do it!! Once o in a blue moon she will hit the pee pad S but but only 1-3”2 times a day after 4-5messes all in different places all the time!! The pee pads have been in same place for one solid year and still pee’s in different spots without even trying to hit the bed!! I gave her a treat yell and scream tell her I love her jumping jacks she knows how happy it makes me I don’t understand why this is gone on for so long I’ve had a check for UTI kidney infection all is well He’s gotten where I can’t even walk in my socks or bare feet I got stuffing in a federal final spot I didn’t see I have to Wn this battle...Unfortunately we do NOT have a backyard or she definitely would be trained already!!
