What Tom would like to know about your dog

Important basic information to gather would be the name and breed of the dog. Next male or female as well as spayed or neutered because it does effect how other dogs will react around them due to testosterone or in heat. Then the age to check for any special needs (seizures, arthritis, cushions, tumors), medications, or strict diets the dog's on.

Other helpful notes would be does the dog get walked if so how many times. U want to keep their routine the same as much as possible. Also how well the dog is socialized such as go to dog parks regularly.
