Is your dog up-to-date on vaccines, and are you willing to provide proof of this prior to booking?
Does your dog have any special medical needs or allergies?
Is your dog friendly with other dogs, puppies, adults, and kids?
Does your dog have any behavioral concerns, fears, or reactive-ness to be aware of?
Is your dog consistently housebroken yet? - this is not required but helpful to know.
What is your dog's method of letting you know when they need to go outside (e.g. ringing a bell, barking/waiting by door, circling, etc.)?
Does your dog have any tendencies such as marking indoors, chewing household items, or counter surfing that we should be aware off?
Is your dog neutered/spayed (not required)? Is it possible that your female dog could be in heat at the time of the booking?
Who is your emergency contact and who is your local vet?
Has your dog ever escaped or attempted to escape before? If so, by what means did/do they attempt this?
How does your dog do on leash and do they have a tendency to chase after or react towards something in particular?
How does your dog do on car rides should transportation or medical services be needed?
Is there any focus on training that you would like me to be consistent with during our interactions?
