What Sara would like to know about your dog
All clients are required to fill out a new client form as well as a boarding contract. The following questions and requirements are included in the policy, but does not replace the contract.
Please be truthful and as comprehensive as possible.
-Color and markings (provide a picture if available)
Please answer the following (positive or negative):
-Invisible fence trained
-Jumps fences or attempts to leave the yard
-Been in a dog fight
-Growls, barks, or lunges at humans, children, or dogs
-Bitten a human, child, or dog
-Attacked another animal
-Pulls on leash
-Bolts out the door
-Has separation anxiety
-Urinates/Defecates in the crate
-Remote (e-collar) trained
-Easy to groom, including nail trimming
-Allows touching of neck, ears, and feet
-Goes into the crate without resistance
-Whines, barks, or growls in the crate
-Guards/growls when you or others approach his food
-Guards/growls when you or others take away his toys
-Growls when you try to remove him from furniture
-Has high prey drive (chases or attacks children, small animals, or livestock)
-Has attacked or killed a child, another dog, or small animal
-Is of wolf, coyote, fox, or other wild canine lineage
-Is sick or injured
Please understand there are always risks when combining dogs from various backgrounds and training. All dogs in the care of BAK9 must have their puppy shots and wearing current identification/rabies tags attached to a secure, flat, nylon or leather collar. Current vaccination records and proof of spay/neuter (from your veterinarian) must be on file at time of service. We will not admit any dog without proper identification/rabies tags, vaccination records, spay/neuter documentation, and signed agreements. All dogs must be healthy and free of any contagious illnesses. Any dog exhibiting symptoms of illness or injury (sneezing, coughing, wheezing, runny eyes or nose, vomiting, lethargy, diarrhea, loss of appetite, limping, etc.) will not be admitted. We are happy to refer local veterinarians with boarding facilities. All dogs must be on a leash when entering/leaving BAK9 and its premises as per city ordinance and for the safety of all animals and humans in the vicinity. Dogs must be potty/house trained (no urinating or eliminating indoors, no chewing, biting, or jumping on people or furniture) and crate trained (no whining, barking, bolting out of crates, or attempting to escape the crate). If they not, they must successfully complete training prior to any boarding services. We feed adult dogs (over 6 months) two-times/day and puppies (under 6 months) three-times/day. If your dog requires a special diet, you (the owner) are responsible for preparing it ahead of time, portioned out for each meal (AM/PM). Any dog left after 3 days of the scheduled pickup without contact from the owner is considered abandoned and will be surrendered to local animal control.
