How much training does your dog have/ what commands do you use?
What activities does your dog like?
Is your dogs normal schedule (feeding and bathroom breaks)?
Does your dog have any fears/triggers to bad behavior?
Does your dog sleep in a crate?
Do you crate your pet when you leave home to go on short trips like grocery shopping?
How is your dog with children?
Has he/she ever met a cat? Not a deal breaker, I like to know so I can make sure I take the right precautions.
Is your pet spayed/neutered and up to date on shots?
Is your dog house trained? Do they mark furniture or tend to drool?
Does your pet have special medical needs?
What kind of food does your dog eat at home?
What are your pet's favorite treats and games?
Does your dog have any behavioral issues such as aggression, digging, escaping, etc? it's not a deal breaker, but I need to know to keep your companion and others safe. Safety is top priority.
The more I know about your companion to make him or her feel at home the better.
