What Pam would like to know about your dog

I need to know what and how often they eat. Do they take any medications? Do they get treats? Do they like other dogs and animals when we walk? How do they respond to other people and children. I need to the name and phone number of his veterinarian. I will need to know where all the supplies are including his leash. To obtain all this information I will provide a complementary cosultation prior to the first visit
