What's their basic info: feeding instructions, the schedule your dog is used to, and their favorite toys and games.

What medical conditions does your dog have? If medication needs to be given, I need detailed instructions.

What's the contact information for your preferred Veterinarian.

What temperament issues, if any, does your dog have? This allows me to better prevent injuries and to keep your dog's visit as stress free as possible.

Does your dog love to jump fences or dig under them? If yes, let me know so I exercise extra caution with them while outside.

What additional tips do you want to share...Do you let your dog onto furniture? What commands or tricks does your dog know? Where does your dog normally sleep?
