Trying to get forms together so when I do dog walking, I can upload or when I do a meet and greet I can hand the forms to the client to fill out or, boarding, day care what ever it may be. Thanks for your help.

I created my own Dog Info Form which I hand out to new clients at our Meet & Greet and they return to me when they drop off their dog. It is just a Word doc, with one side being the critical into (medical, emergency contact, dietary, etc.) and the other is about the dog's behavior and schedule. I searched online for source material, mainly from kennels and other dog boarding facilities. Then decided what I liked to create a custom sheet that met my needs.
I remember others on this saying they used online docs (Google or cloud based) to do that same and no paper was involved. But that's up to you. I have my binder with all the completed info sheets for reference. Easier for me.
