She is 2 years old and has never had accidents in our home or any other home. The only place she has had an accident is at my office, she has urinated in several places and has gone back to the same place twice to poop.
Do the whole house breaking thing all over again for the office. Directly supervise her and when she goes into her pre-potty/pee rituals, take her outside and reward her for doing it in the right place.
It could be due to the anxiety of being in a new place. A dog-friendly office with all it's new smells and people and animals could be overwhelming. Sounds like she's also adjusting to a new routine. Re-potty training is the way to go.
